Tuesday 14 May from 7am until 9.30am ONLINE ONLY: Members with 61 loyalty points or more can purchase one ticket per person.

Tuesday 14 May from 10am until 11.30am ONLINE ONLY: Members with 56 loyalty points or more can purchase one ticket per person.

Tuesday 14 May from 12pm until 1.30pm ONLINE ONLY: Members with 51 loyalty points or more can purchase one ticket per person.

Tuesday 14 May from 2pm until 3.30pm ONLINE ONLY: Members with 46 loyalty points or more can purchase one ticket per person.

Tuesday 14 May from 4pm ONLINE ONLY: All remaining season ticket holders and members can purchase one ticket per person.

Should any tickets remain available then tickets will go off sale at 5pm on Thursday 23 May.

Declaration form submission
Each supporter will need to fully complete their own EUROPA LEAGUE FINAL declaration form once the booking has been made. Each declaration form must have a hand written signature from the individual ticket holder.

The declaration form must be completed accurately including the passport number of the applicant.

Supporters who order two tickets or more in one transaction will be required to submit the application forms from that transaction on the same time.

Failure to comply with the above information will result in the declaration forms being returned without tickets issued.

Supporters who have booked a flight with Thomas Cook Sport will need to submit a declaration form.

Please note the following deadlines are in place for this match:

Declaration forms must arrive with the ticket office no later than 5pm Thursday 16 May.

Supporters who do not receive their voucher must contact the ticket office on 5pm Thursday 23 May.

Collection Procedure
Once the fully completed declaration forms have been received a voucher will be posted to the supporter who made the booking.

In order to collect the ticket each individual supporter will be required to bring with them in person the voucher and their valid passport. Please note copies of the passport will NOT be accepted and each supporter will need to visit the collection point in person. Tickets cannot be collected on behalf of other supporters.

Supporters who purchase a ticket at the ticket office in person will have the voucher sent out by post. No vouchers will be handed out at the ticket office at the time of purchase.

Further details in relation to the location of the ticket collection point, along with match security and safety information will be communicated to supporters by email as soon as possible.

Supporters who have booked a flight with Thomas Cook Sport will need to collect their match ticket at the ticket collection point.

Business season tickets or multiple season tickets in one name
Season tickets held in a business name (as determined by the Club) cannot purchase tickets for UEFA away matches.

Any season ticket holder who holds two or more season tickets in one name can only purchase one UEFA away match ticket.

Tickets purchased using season tickets in a business name or multiple names will have the ticket order refunded.

Duplicate tickets
We regret to inform supporters there will be no duplicate tickets available for this match.

Supporters will be required to have a visa to enter Azerbaijan and should apply no later than 20 May 2019. An e-visa has a single-entry status and is valid for 90 days with 30 days permission to stay in the country. The e-visa will cost 20 USD (US dollars) plus a 3 USD service fee. Official travel
Thomas Cook Sport will be offering supporter travel to the Europa League final in Baku.

Day trip places will be offered to supporters who have purchased an official match ticket from the club (subject to availability).

All applications to travel on the official trips will be verified by the club for a match ticket purchase, prior to being confirmed by Thomas Cook Sport. Match tickets need to be collected from the ticket collection point. A fully completed declaration form will need to be submitted to Chelsea in line with the usual ticket collection procedure.

These trips are only available to supporters in possession of an official match ticket. All applications will be verified by the club for a match ticket purchase before being confirmed by Thomas Cook Sport. Any applications from supporters advised to us by the club as not in possession of a match ticket will be cancelled.

All supporters on your travel application must have an official match ticket in order for the full application to be confirmed. Any applications where any supporter does not have a match ticket will be cancelled in their entirety. Therefore, if you want your application to be considered individually, please make individual applications.

We will accept requests to link applications onto the same flight, however we can’t guarantee that this will be possible. Please advise of the Thomas Cook Sport reference number of the supporters you wish to be linked with in the ‘Travel Special Request’ field during the booking process.

We will be oversubscribing applications for this trip. Confirmations will be done in order of match ticket purchase, as instructed to us by the club. By applying for travel, you are registering your intent and are in agreement that we can take payment from your card and confirm your booking once we are advised you are eligible by the club. All applications are subject to availability until a booking confirmation showing a ‘confirmed’ status is received by the lead applicant, by email. Once confirmed, you will be subject to our terms and conditions.

Applications will be open until 5pm on Wednesday 15 May.

We reserve the right to end applications without notice. Any applications not made by this time will not be taken, regardless of the supporters’ level of membership. Should further availability arise, we will re-open applications and these applications will be confirmed in line with the clubs selling criteria, subject to availability, until applications close.
