Supporters attending Saturday's Premier League game should read the below information on arrangements at the Etihad Stadium and the declaration form that must be completed in advance...
Chelsea supporters aged 18 or over attending the game will need to complete a self-declaration form confirming their Covid status ahead of the match.
The form can be found below and this online process requires fans to confirm they have a valid NHS Covid pass or negative NHS Covid test within the previous 48 hours.
Following the Government’s introduction of ‘Plan B’ COVID-19 measures in England, the Premier League has implemented new protocols for all supporters attending matches.
To attend a match at the Etihad Stadium supporters must ensure they have either a valid NHS COVID Pass on your mobile device, proof of a negative Lateral Flow COVID-19 test result (SMS or email, which has been registered with the NHS) taken within 48 hours of the matchday, or evidence of exemption.
SUPPORTERS OVER THE AGE OF 18 MAY BE ASKED TO PRESENT PROOF OF THEIR COVID-19 STATUS BEFORE ENTERING THE STADIUM. IF YOU ARE UNABLE TO DO SO WHEN ASKED, YOU WILL BE DECLINED ENTRY TO THE STADIUM IN LINE WITH LEGAL REQUIREMENTS, UNLESS YOU ARE EXEMPT.
Remember that you must not attend a match if you are experiencing COVID-19 symptoms or if you are at risk of passing on COVID-19.
