Entries close at 11.59pm (UK time) on Thursday 17 October.
Winners must be able to attend the event at Stamford Bridge from 5.30pm – 7.30pm on Thursday 7 November.
Please note all under-18s and vulnerable adults must be accompanied by a parent/guardian to the event (18+).
