Business season tickets or multiple season tickets in one name

Season tickets held in a business name (as determined by the Club) cannot purchase tickets for UEFA away matches.

Also any season ticket holder who holds two or more season tickets in one name can only purchase one UEFA away match ticket.

Tickets purchased using season tickets in a business name or multiple names will have the ticket order refunded.

Declaration form submission

Each supporter will need to fully complete their own declaration form once a booking has been made. The declaration form can be downloaded from the website. Supporters who order two tickets or more in one transaction will be required to submit together the declaration forms for all supporters who form part of that transaction.

However each form must be signed and certified as correct by the individual ticket holder.

IMPORTANT – The declaration form must be completed accurately including the passport number of the applicant.

Failure to comply with the above information will result in the declaration forms being returned without tickets issued.

Declaration forms can be submitted in one of the following three ways:

Email – uefa.away@chelseafc.com remembering to attach the declaration forms.

Personal Caller – by visiting the ticket office in person with the completed declaration forms. Please note the vouchers will be sent out by post and not handed out at the window.

Post – Post it to PO Box 21805, Fulham Road, London, SW6 1HS.

We recommended that applications being sent by post should be done so using Royal Mail Special Delivery.

To download a declaration form please click here and select the tab downloadable forms.

Collection Procedure

Once the fully completed declaration forms have been received a voucher will be posted to the supporter who made the booking.

In order to collect the ticket each individual supporter will be required to bring with them in person the voucher and their valid passport. Please note copies of the passport will NOT be accepted and each supporter will need to visit the collection point in person. Tickets cannot be collected on behalf of other supporters.

Supporters who purchase a ticket at the ticket office in person will have the voucher sent out by post. No vouchers will be handed out at the ticket office at the time of purchase.

Further details in relation to the location of the ticket collection point, along with match security and safety information will be communicated to supporters by email as soon as possible.
